The options discussed on this page are found by selecting Menu --> Setup --> Communication --> Online Forms

‘FLEX Forms’ allow web forms to be designed and sent to the patient for remote pre-screening / triaging. You can build your own form here - no programming experience is needed. 

To create a form, click to Menu - Setup - Communication and choose 'Online Forms'. Here, you can Create, Update and Delete Online forms.

Click New to make a new form. You can tick the form as Active, and give it a Title and Intro. You can also add an Image which the patient will see - perhaps your practice logo. You can enter a Form Link which will mask the URL (unique link) sent in the SMS / Email to them - or leave this blank for the link to be 'as is'. 


Available Controls shows the 4 different aspects which you can add to your form. 'Separator' is purely to put a line between aspects. The controls are:


Edit - Allows the patient to free-type text

Combobox - Allows the patient to select a single answer from a pre-set list

Radio buttons - Allows the patient to make a single choice from a series of options

Check boxes - Allows the patient to make multiple choices from a series of options


Press + Add to add these to your form. This is shown below.


You can set the order of questions using the arrow boxes at the far end of each control, or use the red minus symbol to remove the question. You can enter your Question in the box on each section and (barring Edit Control) set the number of # Options, You can then double-click into the Options box to set your answers.


Click Save when you are happy with your work.


We are almost ready to send the patient a link in either an email or SMS. You can link a form to an appointment confirmation and let FLEX send the link automatically, by either SMS or Email. See How Do I Set Up Appointment Reminders? for help with this. 


You will need to add your form link into your SMS / Email message. To do this, in the SMS / Email template of your choice (Email is used in the example below). In the Patient database, double-click or drag and drop one of the two highlighted fields into your template. Only the long version can be used in SMS templates. Once happy, save your template.


The Form data is sent to the patient and downloaded back into FLEX for review through the Px Record. On the Patient Record, press 'Message' and choose Online Forms.


To send to the patient, press 'Send Link to Patient' and choose your Form Template and then Email / SMS Template from the list. To be able to send, three criteria must be completed:


  1. You will be prompted at this point if you haven't added the URL into your SMS / Email template
  2. The patient will need to have an email address and/or mobile number (the patient in question has no mobile in the example image below, hence SMS is greyed out)
  3. You will need to use SMS / Emails through FLEX - see Email and SMS for more help.


To see submissions, press 'Message' on the patients record and choose Online Forms . View Submissions. Double-click to open and view, or right-click to delete. An example of a completed template set to a patient is shown below.
